A hydrocele is an excessive collection of fluid in the space around the testicle. In adulthood, a primary hydrocele may develop from an imbalance between production and reabsorption of the normally very small amount of fluid present. It is usually a painless swelling but may be felt as ‘heaviness’, particularly if large in size. Surgery can be performed if the swelling is causing symptoms and/or has become large. A secondary hydrocele occurs when fluid builds up around the testicle due to an underlying disease in the testicle such as infection, trauma or a tumour. Treatment is directed at the underlying problem in the testicle.
